Holistic Needs Assessments (HNA's) empower people to express what matters to them, at various points in their cancer pathway. We work with our Cancer Nurse Specialist teams and Cancer Care Co-ordinators in finding solutions, creating processes and improving patients experience by embedding personalised care in everyday practice. 

The programme also supports the implementation of End of Treatment Summaries alongside Personalised Stratified Follow-Up`s (PSFU). The Living with Cancer team have supported our acute clinical teams with education and skills development to improving their connections with local community services thus ensuring patients are supported closer to home. 

The Rapid Diagnostic Concept - RDC - focuses on introducing pathways which deliver faster, earlier diagnoses and improve patient experience. RDC’s will offer a personalised, accurate and rapid diagnosis of a patient’s symptoms.

The Screening and Early Diagnosis programme focuses on improving uptake within three national cancer screening programmes. Earlier and more timely diagnoses significantly impacts and benefits the lives of those living with or affected by cancer.  This programme addresses and works to reduce existing health inequalities across Lincolnshire.
