What is a Next Steps Clinic?

Upon finishing treatment, you will be invited to a Next Steps Clinics, where one of your cancer team will provide you with advice on community support and how to access it. 

During your Next Steps Clinic, your healthcare professional will discuss your holistic needs and will carry out a Holistic Needs Assessment, this helps to identify areas where you feel you may need more support. It is important to talk to your health care professional about any worries that you might have; understanding what affects you the most will help your care team find the most suitable community support for you. The Holistic Needs Assessment will then form part of your Care Plan. 

What is the T.R.R.N Model?

Within the Living with Cancer team, we are always looking for communication tools, to better support people living with cancer in Lincolnshire, in identifying and understanding their support needs.

Some we have developed ‘in house’ and others we have resourced from the wider health setting:

What is PAMS - Patient Activation Measures?

An example of a resourced communication tool,  is the Patient Activation Measure, a questionnaire, that identifies how able or willing someone is to self-manage and or self-care, below is a graphic which gives an insight into the questionnaire.
